Repeating the same story to the many health professionals who need to be consulted, contacting services, monitoring that appropriate care is delivered, searching for the right residential facility

A case manager is an expert with an overview of a person’s needs, and a facilitator of appropriate care and service.

Australian Dementia Support can provide, on a contract basis, a case manager for you or your loved one.

Our case managers are experts in the care of dementia sufferers. Whether at home, or in a residential facility, a contracted case manager will regularly monitor the care needs of the client, be a ‘sounding board’ for carers, and make referrals or recommendations for appropriate services.

This service is particularly helpful for working carers, or those separated by distance
